For decades JFS has provided services and referrals that help adults ages 60 and older age-in-place, support caregivers and improve quality of life. Under the leadership of Senior Services Director Debbie Pozniakoff, a team of case managers ensures that you have the tools and resources needed to support safe living at home and enrich quality of living as you continue to navigate new stages of life. During this time of medical risk and prolonged isolation, JFS services and client relationships are as important as ever.

JFS case managers collaborate with you to develop and manage an individualized care plan that facilitates and supports continued independence and safety in your desired living environment. We provide resources and program referrals, subsidized in-home support services, falls and hospital re-admission prevention, and counseling and friendly visits as needed.

Elderlink, a telephone-based assessment, information and referral service, is staffed by a JFS case manager to assist callers and assess needs. Callers can request and be provided with resources and referrals relevant to their needs. All clients are offered a comprehensive assessment that will evolve into a personalized care plan.

At the initial home visit (currently virtual or by phone), assessments are completed and a comprehensive and holistic care plan is developed to address your individual needs.

JFS supports independent living through a network of referral partners that offer transportation to medical appointments, food delivery, utility assistance, minor home repairs, home health care, housing information, support to apply for government and veterans benefits and Medicaid, socialization opportunities, and more. Clients may also receive psychoeducation on a disease process, patient rights information, mental health and substance abuse treatment, and home safety information. Through years of professional networking and collaboration, JFS staff have built relationships with numerous organizations, including hospitals, senior living centers, hospice providers, and durable medical centers.

 

Changes to Services Delivered:

The COVID-19 crisis has required some modifications to our services. In response to the current global pandemic, home assessments and case management services are being carefully and caringly conducted through Zoom for Healthcare or by telephone. All community presentations have been canceled. JFS will continue to follow recommended public health and safety practices to determine when to return to home visits.
